Behavioral Services Expectations

What you can expect from me:

  1. l will schedule and attend meetings. l will provide 24 hrs notice if a session must be canceled. On rare occasions, 24 hrs notice may not be possible if the missed appointment is due to illness or an emergency. l will do my best to provide as much notice as possible
  2. l will return phone calls within 24 hours
  3. l will develop a behavior intervention plan and review the behavior plan with caregivers and staff. l will address any concerns or suggestions from caregivers and/or staff.
  4. l will model and teach procedures listed in the behavior plan.
  5. l will provide data sheets and review target behaviors and data collection procedures.
  6. l will graph data collected by caregivers and/or staff
  7. I will review data and graphs with caregivers and/or staff
  8. l will answer caregiver and staff questions regarding treatment to the best of my ability.
  9. l will provide notice before terminating services.
  10. l will comply with HlPPA regulations.

Your participation is vital to the success of the program. Therefore, the following are
required:

  1. Notify the behavior therapist by phone if an appointment must be canceled. Please try to give 24 hrs notice when possible.
  2. Attend scheduled appointments.
  3. Ask questions and express any concerns regarding treatment.
  4. Cooperate with the behavior therapist's efforts to provide services by following through with any interventions recommended by the behavior analyst. If you need further assistance or have concerns, please do not hesitate to discuss your needs with the behavior analyst.
  5. inform the behavior analyst of any changes in diets, medication or the addition of other treatments prior to the onset of these changes or immediately after.
  6. Collect data on problem behavior and replacement behaviors (e.g., accepting "no," requests, etc...) identified by the behavior analyst. Data is used to determine the effectiveness of treatment and is a necessary component of behavioral services.


Failure to comply with the items listed above could result in the termination of behavioral services.
